New Patient Registration

registrationThe doctors welcome new patients who live within our practice area. 

Medical treatment is available from the date of registration.  Please contact reception for further information.People living within the Practice area are able to register with the surgery.

You can register by either completing a registration form available at the reception or by using the online facilty.

You will be required to provide us with one proof of identity.

Your details will be entered on our system and you details will be forwarded to the local Health Authority. All patients will be allocated a named GP, though you are free to see any other doctor if preferred. New patients will be asked to complete a simple questionnaire, following which a basic health check will be carried out. Medical notes often lag behind when a patient moves from one practice to another and these checks help us to provide a more efficient service to you.
